TRIOS vs. iTero in 2026: How the Two Leading Intraoral Scanners Compare

Shopping for an intraoral scanner often comes down to two names: 3Shape TRIOS and Align iTero. Both scan crowns, implants and aligner cases well. The differences sit around the wand: which aligner system accepts your scans, how cases reach your lab, which diagnostic aids are built in, and whether prices are published.

How Fast and How Accurate Are They?

Speed figures are manufacturer claims measured differently: 3Shape counts images per second and claims up to 110% higher resolution for TRIOS 6 than the rest of its portfolio; Align quotes full-arch times and a 3x wider field of view for Lumina than for Element 5D. For accuracy, Align cites its own July 2024 bench test (data on file) in which Lumina beat TRIOS 5 and three other scanners. Independent studies that tested both brands are mixed:

In a systematic review of 13 studies of complete-arch implant impressions that included older iTero and TRIOS models, photogrammetry beat intraoral scanning in 10, and the authors still recommend a rigid prototype try-in (Pozzi et al., 2025). Training also improved scan trueness by about 19 µm (Limones et al., 2026).

Our reading: for single units and quadrants, both brands are accurate. For full-arch implants, Lumina has led TRIOS 5 and 6 in most studies, all on lab models. In patients, TRIOS 4 deviated less than Element 5D, so generation matters as much as brand.

Which Caries Aids Are FDA Cleared?

  • iTero NIRI passes 850 nm near-infrared light through the tooth: healthy enamel looks dark, dentin and lesions bright. It is cleared as an aid for detecting and monitoring interproximal caries above the gingiva: Element 5D in 2020 (K193659), Lumina Pro in 2024 (K240573) and a modified Lumina Pro in July 2026 (K261847).
  • 3Shape Dx was cleared on April 10, 2026 (K260082) as an aid for caries, tooth wear, gingival recession and plaque; 3Shape says the clearance covers adult patients. The AI version, Dx Plus, is a subscription that works only with TRIOS 6 scans, and its proximal caries view is not AI-detected. TRIOS 5 was cleared in 2022 as a fluorescence caries aid (K221249).

Independent data are limited. In 66 adults, Element 5D NIRI found 81.8% of the lesions that bitewings showed in dentin, with 93.9% specificity, but the authors say it does not replace bitewings when staging is needed (Bittar et al., 2026). Against histology, NIRI sensitivity was 37%, against 52% for bitewings (Rodrigues et al., 2026). TRIOS 5's fluorescence mode agreed poorly with panoramic radiographs and visual scoring (Öztürk Tunçel et al., 2026). We found no peer-reviewed study of Dx Plus in PubMed as of September 2026.

Invisalign and Orthodontics: The Biggest Divide

iTero is built around Invisalign: every current model lists Invisalign integration, case and progress assessment tools, and automatic scan upload for case submission.

TRIOS is the opposite in the US. 3Shape's support page lists the United States among the countries where TRIOS scans are not approved for Invisalign, and its TRIOS 6 FAQ says TRIOS 6 does not integrate with Invisalign at all. TRIOS users still have options: 3Shape Unite lists more than 100 partners across clear aligners, 3D printing, milling and other services.

Open or Closed: How Do Scans Reach Your Lab?

Both are open at the file level. TRIOS outputs PLY, DCM and STL, and 3Shape says scans can go to the lab of your choice; labs registered with 3Shape connect in one click. iTero exports STL and PLY, routes cases through the MyiTero portal, free for labs, and connects directly to exocad; Align puts mean transfer time at 4.6 minutes, based on 10,000 scans.

What Do Software and Service Plans Cost?

  • TRIOS: 3Shape Unite is free, and 3Shape says TRIOS 6 has no mandatory subscription. The first year of TRIOS Care is included. After that, TRIOS Care starts at $219 per month (unlimited support, 5-year warranty, drop coverage, 10 TB of cloud storage), or TRIOS Only costs $0 with support and repairs paid per case. Dx Plus is extra.
  • iTero: the service plan covers remote support, hardware replacement, software and OS updates, unlimited scans and unlimited cloud storage. Align says a separate plan is required after the first year unless an extended plan is bought up front, and it publishes no plan price.
